WHAT ARE VISITING HOURS?
Flexible. We invite family and friends to join activities as well as have meals with us. Arrangements for meals should be made two hours in advance. Immediate family or other relatives may visit at any time; however, it is requested that visits prior to 10:00 a.m. or after 8:00 p.m. take place in the Facility’s common areas to assist care giving and protect the privacy of other residents, if necessary.
WHAT DOES THE DAILY ROOM RATE COVER?
The daily charge includes room, board, personal care and telephone. Medications, medical supplies, physician fees, TV and barber/beauty charges are extra.
IS THERE A PHONE IN EACH ROOM?
Yes. Every room has a phone that is shared by each person in the room. Private lines are available through Verizon at the resident’s expense.
I HAVE ALL MY MEDICATIONS. CAN I BRING THEM WITH ME?
No. The charge nurse will order and dispense medications according to your physician’s orders. Please see Admission Director for special circumstances.
WHAT CAN I DO TO MAKE THE ROOM MORE HOMELIKE?
As long as a wheelchair can pass through the room, we have no objections to your favorite chair, bedside lamp, pictures, etc. We welcome a personal touch.
WHEN DO I APPLY FOR MEDICAID?
When you have approximately two months of private pay funds remaining.
ARE THERE ANY CHANGES WHEN I BECOME A MEDICAID RESIDENT?
There are no changes in patient care.
MAY I SMOKE AT SUNDALE NURSING HOME?
No. Due to the county regulations we do not have a location for residents to smoke. Our physician can discuss the option of a Nicotine patch.

WHAT DO I WEAR?
Seasonal street clothes. We get our residents up and out of bed as much as possible.
DO YOU DO MY LAUNDRY?
Yes, however families may choose to do the Resident’s own laundry or have someone do it. In every case, please have each item marked with the Resident’s name.
DO I NEED SPENDING MONEY? IF SO, HOW MUCH DO I KEEP?
Yes, if you would like to purchase small items for example from the vending machines. IF you go on outings with the activity department, you will also need your own money. Funds can be kept in the business office for safekeeping, by opening a patient fund account, which is an interest bearing account. Patient fund can be utilized to pay your monthly account balance.
DO YOU HAVE ACTIVITIES?
We offer a variety of group as well as individual opportunities, both inside and outside of the facility. Activity calendars are posted in the facility and in resident rooms.
IS THERE A TV PROVIDED IN EACH ROOM?
No. We encourage families to provide portable TV’s with remove control. There is an extra charge for cable service.
DO YOU HAVE A NEWSPAPER DELIVERY?
The facility receives newspapers daily for the residents to read. They must share with others. If they want their own paper, you can arrange that through the Dominion Post.
WILL I ALWAYS KEEP THE SAME ROOM?
The compatibility of roommates is important. Therefore, there might be a time, due to differing personalities, behavior, or illness that a room change would be necessary.
DOES MEDICARE PAY FOR MY STAY?
Medicare will pay for skilled nursing services if qualifications are met. They cover days 1-20 at 100 percent. Days 21-100 Medicare pays everything except a co-payment of $170.50 a day. Medicare does not guarantee 100 days. This depends on qualifications or needs.
HOW DO I APPLY FOR MEDICAID?
The Director of Social Services will be pleased to give you the phone number to call for an application.
WHEN I AM READY TO GO HOME HOW MUCH NOTICE DO YOU NEED?
We require notice of 7 days prior to discharge. This gives Social Services the opportunity to get the medications ordered and any equipment that you may need.
