COVID-FREE FACILITY
NOTICE OF MANDATORY COVID-19 REPORTING
BY ALL NURSING FACILITIES TO THE CDC
CMS (Centers for Medicaid and Medicare Services) has mandated all nursing facilities to report weekly COVID-19 data to the CDC (Centers for Disease Control and Prevention) effective May 1, 2020.
Please be aware that we will be reporting the following information weekly to maintain compliance with the CMS directive:
• Suspected and confirmed COVID-19 infections among residents and staff, including residents previously treated for COVID-19
• Total deaths and COVID-19 deaths among residents and staff
• Personal protective equipment and hand hygiene supplies in the facility
• Ventilator capacity and supplies in the facility (does not apply to WV nursing facilities)
• Resident beds and census
• Access to COVID-19 testing while the resident is in the facility
• Staffing shortages

Welcoming Visitors
Families and visitors are welcome anytime. We do suggest the hours from 9:00 a.m. to 8:00 p.m. Children of all ages may visit. We encourage you to make use of our attractive dining room, lounge, and patio during visits. We also will be happy to assist you with special occasion gatherings, such as birthday and anniversary parties, so feel free to notify our Activities Department.
Food from home must be checked in with the charge nurse before given to the resident to determine compatibility with the Resident’s diet/medication regimen. Please do not leave perishable items or amounts that cannot be readily consumed with residents. Snack food must be stored in sealed plastic containers and dated.

We encourage visitors to eat with our residents. Please notify the Receptionist or the Dietary Manager when you plan to have a meal. There is a charge of $3.50 per meal.
